In case of Bereavement
If a death occurs in the family at home, please inform immediately the patient’s doctor who will need to confirm the death. You may also inform a funeral director of your choice, who will be able to help with practical details.
If death occurs in hospital, please notify your doctor as sometimes communications between the doctor and the hospital can be slow. We do not wish to cause unnecessary grief to relatives. The patient’s doctor will usually be able to issue a death certificate for the Registrar of Births and Deaths, and a cremation form if required. In certain cases of sudden death, the doctor will be required to inform the Coroner’s office at the police station.
Everyone reacts differently to bereavement. It is an experience we will all have to cope with at some time in our lives, but it is nonetheless a severe form of stress. Your doctor will be a willing listener and source of advice.
Zero Tolerance
We strongly support the NHS policy on zero tolerance. Anyone attending the surgery who abuses the GPs, staff or other patients be it verbally, physically or in any threatening manner whatsoever, will risk removal from the practice list. In extreme cases we may summon the police to remove offenders from the practice premises.
Freedom Of Information - Publication Scheme
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available. This scheme is available from reception.
Computers
Your registration details and your medical history details are held on our computer. The computer produces all the repeat prescriptions and helps us to keep accurate data about your medical care. If you have any questions about the computer, the doctor or practice manager will be happy to help. We are registered under the Data Protection Act.
